Enter the information exactly as your provider has it on file. ABS requires an exact patient match; it does not show possible matches.
What must match: provider office, legal first name, legal last name, and date of birth. Then enter at least one supporting identifier: ZIP code or the last four digits of a phone number on file. You may enter both if you know them. For privacy, ABS will not tell you which field did not match and will not display possible patient records. Repeated unsuccessful attempts are temporarily blocked.
